REAP: Referral, Education, Assistance & Prevention

Good Morning Jackson!

Are you having difficulty dealing with a loss or multiple losses in your life?  Having trouble sleeping at night?  Difficulty managing your medication?  Don’t risk your independence, ask for help from the R.E.A.P.

R.E.A.P. stands for Referral, Education, Assistance and Prevention.  This is a free service, available to seniors 60 years of age or older AND caregivers or family members, to support senior independent living.  R.E.A.P. counselors can provide over the phone help or even home visits if needed.  It is a free and confidential service administered through Northern Human Services.  Life can be troubling at times, there is no need to “go it alone”.

For more information, or to connect with a R.E.A.P. counselor you can call ServiceLink  at 1-866-634-9421 or go on line at dhhs.nh.gov/dcbcs/bdas/reap.htm
